Early Orthodontic Treatment

Proactive. Simple. Smart.

The American Association of Orthodontists recommends that children first see an orthodontist as early as age seven.

Early treatment—also known as Phase I—typically begins between ages 7-9, while Phase II will begin around age eleven or older. At your first visit, Dr. Scarborough will evaluate your child’s growth and development and provide you with a detailed plan for your child’s treatment. In many cases an early phase of treatment is not needed. In these cases, Dr. Scarborough will follow your child’s growth and development as much or as little as you desire to insure proper balance between the jaws and the teeth. This is a service provided to you as a courtesy, at no cost to you. Please ask us about our Kids Club and any upcoming events!

The goal of early treatment is to develop the jaws to allow the most ideal dental development and create the best foundation for your child’s future bite and smile. Enhancing the growth of the jaw and correcting certain bite concerns early, such as a underbite, crossbite or crowding, eliminates more aggressive lengthy forms of treatment including permanent tooth extractions and surgery.

Other orthodontic problems such as crowding of the teeth, too much space between the teeth, jaw growth problems, protruding teeth, early or late loss of baby teeth, or thumb sucking habits are all scenarios in which children can benefit from early treatment. Additional advantages include: increased self-esteem and improved breathing (expansion treatment).

If your child is between the ages of seven and nine it is an ideal time to get your orthodontic plan. Referrals are not necessary and complimentary exams can be scheduled directly with Dr. Scarborough at James River Orthodontics. An early assessment of your child’s growth will give you the peace of mind that your child is thriving. Our highly trained and energetic team will provide your and your child with a fun and informative initial exam, and will discuss with you the best steps to take toward caring for your child’s smile.

Two Phase Orthodontic Treatment

Beautiful. Healthy. Forever.

Two-phase orthodontic treatment is a specialized process combining tooth straightening and physical, facial changes.

The purpose of two-phase treatment is to maximize the opportunity to accomplish the ideal healthy, functional, aesthetic result that will remain stable throughout your life. Early treatment is most effective for achieving lasting results.

Phase I (Building a Strong Foundation – Smile Development)

The goal of Phase I treatment is to enhance the jaw develop in a way that will accommodate all of the permanent teeth and create the best foundation for the jaws to grow in harmony with each other. The healthiest and most beautiful smiles start with great beginnings.

Phase I is accomplished in 8-14 months and followed up by a resting period. Retaining devices are used short-term and are eliminated when permanent canines begin to erupt. It is best to allow the existing permanent teeth some natural freedom of movement to best settle into the bite making Phase II highly efficient. A successful first phase will have created room for permanent teeth to find an eruption path and developed the jaws to enhance the way in which the jaws grow and relate to each other. This creates the best foundation for the permanent teeth with regard to aesthetics and function.

At the end of the first phase of treatment, teeth are not in their final positions. This will be determined and accomplished in the second phase of treatment once all permanent teeth have come in. Timing is critical, and therefore, periodic recall appointments for observation are necessary, usually on a six-month basis.

Phase II (Perfection Stage – Smile Design)

The goal of the Phase II is to make sure each tooth has an exact location in the mouth where it is in harmony with the lips, cheeks, tongue, and other teeth. When this equilibrium is established, the teeth will function together properly and stability is increased. Dr. Scarborough prides himself in aesthetics and takes significant care in designing each individual smile. Phase II usually involves full braces, and many times can be accomplished with Invisalign® as the heavy lifting was completed in Phase I.

Phase II begins when all permanent teeth have erupted and is completed 12-24 months. Retainers are worn after this phase to ensure you retain your beautiful smile.

Retainers

Retainers are used once your teeth have reached their ideal position in order to hold them and keep them from moving. JRO offers Retainers for Life™, to insure you continue to wear them and are not burdened by additional fees. Please ask us about this unique to JRO and unbelievable program!

Dr. Scarborough may recommend a specific type of retainer for you, or he may give you the option of choosing one that you are most comfortable with. At the beginning of your orthodontic treatment we will spend time discussing all of your retainer options with you in order to make sure your smile stays beautiful for the rest of your life!

Surgical Orthodontics

In some special cases, surgical orthodontics may be required to correct problems such as jaw bone abnormalities, bad bites and malocclusion. 

Dr. Scarborough, a former member of the Medical College of Virginia, Department of Orthodontics Craniofacial/Facial Reconstruction Team, has extensive experience coordinating interdisciplinary and complex cases that combine orthognathic jaw surgery with orthodontic treatment.

A portion of your initial consultation will be spent determining if this is necessary for your individual case. If it is decided that surgery is necessary, we will work with you to ensure that you receive the best care possible and your result restores both function and aesthetics to your smile.

TMJ Treatment

Function. Harmony. Health.

Many people suffer from chronic facial and neck pain as well as severe headaches. In some cases this pain is due to a Disorder of the TMJ, also known as TMD.

Your temporomandibular joints, or TMJs, connect your lower jawbones to your skull. These joints get much use throughout the day as you speak, chew, swallow and yawn. Pain in and around these joints can be unpleasant and may even restrict movement

Symptoms of TMD include:

  • Pain in the jaw area
  • Pain, ringing or stuffiness in the ears
  • Frequent headaches or neck aches
  • Clicking or popping sound when the jaw moves
  • Muscle spasms in the jaw area
  • A change in the alignment of top and bottom teeth
  • Locked jaw or limited opening of the mouth
